What type of data will be collected?
First, the facility champion will answer several questions about the facility and its resident population. Next, the facility champion will enter blinded data from 20 distinct resident charts AND 10 staff members (staff members can include employees, volunteers, and independent practitioners) into a data collection form. One data collection form will be completed for each resident chart or staff member for a total of 30 data collection forms at each stage.

I am a nurse at one of the participating facilities – can I also complete this activity?
Yes, the full healthcare team is encouraged to participate! This activity will review processes, which often affect everyone involved in the delivery of care to residents.

Who will be conducting the in-service meeting at my facility?
Physicians within the state of Indiana will provide the in-service meeting at your facility. They will review data collected in Stage A (information that you upload into the program portal) and will analyze and compare the data to national targets. The speaker will develop a custom presentation based on these findings.

What do we need to provide for the meeting?
The facility champion will need to upload information onto the program portal in order for the faculty to develop a custom presentation.

The champion will need to coordinate with Katie Hacias at ACHL a date that is available for the speaker to present to staff at your facility, and reserve/coordinate a meeting location.
